Five Note Audit

Paste one to five de-identified notes, office visits or operative notes in any mix. The AI codes each one. Enter what you coded, and it shows exactly what the difference is worth.

Please remove all patient identifiers (PHI) before pasting; details at the bottom of the page.
Optimized for: any mix of the above; each note is routed to the matching engine.
Full access: full clinician access, built for normal daily clinical use: $199.99 a year or $19.99 a month. Get full access Sign in
Yearly projection (optional). To turn a per-note difference into a yearly estimate, enter your averages; the audit multiplies the average missed value per note across a year of visits: clinic visits per week, surgeries per week, weeks of vacation. Clinic notes project against clinic visits; operative notes project against surgeries.
Free monthly coding email
One short email a month: a commonly missed code pattern, plus any CMS value or bundling changes that matter. Unsubscribe anytime.
About this tool: privacy, PHI, pricing, and using it at work

De-identified text only; PHI is prohibited and no BAA exists. Pasted text travels encrypted to the AI provider solely to produce the analysis; the raw note is never written to persistent storage operated by this site (the analysis is held briefly in temporary job storage, deleted on retrieval, expiring within an hour), submissions are never used to train models, and provider retention is limited before deletion. Each run counts against the shared free allowance of ten analyses per device; full access removes the limit. Educational support only: code what your documentation supports, per your organization's policy. Full privacy details.

Free to try: 10 analyses on this device. Full access is full clinician access, built for normal daily clinical use: $199.99 a year or $19.99 a month. Get full access Already purchased? Enter your code on the E/M Assistant; it unlocks every tool on this device. Or sign in to use full access on any device.
How to use this at work: keep this page open in a pinned tab beside your chart. Copy each note and remove all identifying information before pasting, not just the name, date of birth, and record number: also dates, facility and clinician names, locations, and any detail that could identify the patient, per your organization's privacy and AI policies. Then run the audit and use each note's rationale to fix the pattern going forward.
