Modifier multipliers reflect common payment conventions: bilateral (50) at 150 percent, second procedure (51) at 50 percent for payment, co-surgeon (62) at 62.5 percent each, assistant (80 and 82) at 16 percent; modifier 22 requires documented substantially increased work and payer approval, commonly recognized at roughly 20 to 25 percent more, shown here at 25. Employer wRVU crediting policies vary, especially for modifier 51; confirm how yours credits reduced-payment lines. Codes marked verify have a value pending fee schedule confirmation and count as zero in the total. Value provenance, stated plainly: most codes carry values taken directly from the CMS Physician Fee Schedule 2026 file; codes beyond that practice file carry values derived from a prior published VA fee table converted to the 2026 scale at a factor validated against 358 overlapping codes, and derived values are estimates pending direct confirmation against the CMS file, not directly verified 2026 figures.
